Your Role: Driving Independence & Growth!
* Build Your Caseload: Take the lead in actively building and sustaining your participant list, leveraging both our referral pipeline and your own networking skills.
* Provider Network Mastery: Act as a \"linker\" by maintaining and expanding a robust network of local providers across Newcastle and Maitland to ensure bespoke support solutions.
* Results-Focused Coordination: Focus on capacity building and self-advocacy, helping participants reduce their reliance on formal supports over time.
* Local Presence: Conduct client visits (generally within 30 minutes of your location) to maintain high-quality, face-to-face relationships.

_ Requirements:_
* Experience: Minimum 2 years in NDIS Support Coordination or Case Management.
* Qualifications: Tertiary degree in Social Work, Community Services, Psychology, Disability Studies, or a related human services field.
* Business Essentials: ABN, Public Liability & Professional Indemnity Insurance.
* Compliance: NDIS Worker Screening Check, NSW Working with Children Check, Driver’s Licence, and a comprehensively insured vehicle.
