Adult Community Health (ACH) provides timely and appropriate clinical services to members of our community to support patients return to independence and facilitate self-management of complex health needs. Our focus is providing wrap around services to the acute sector to reduce admissions, support timely discharge and developing effective links to primary health care providers to encourage our clients' sustainable self-management in the community.
About You:
* Provide leadership to effectively plan, manage and evaluate multidisciplinary adult community health services to achieve organisational goals across the service.
* Monitor and meet Key Performance Indicators (KPI's) defined for the Adult Community Health Services.
* Effectively manage self and collaborate with professional resources within the Division and HHS, to develop staff to enable fulfilment of role responsibilities and portfolios.
* Provide effective communication, consultation and negotiation throughout the service to ensure the involvement of stakeholders.
* Team Leader provides oversight to the Clinical Leads to effectively lead and coordinate quality improvement and evidence-based practice activities to ensure the provision of quality, cost-effective services.
* Accountable for the application of contemporary human resource management principles to ensure that the unit working environment meets legislative standards and Queensland Health policy.
