(Pro Rata) RNM2C| Fulltime- 12 Month Contract Available Join our Marion Psychosis Team Work collaboratively with consumers and families/carers in the community. Our Psychosis Stream is responsible for delivering high-quality, recovery-oriented clinical care to consumers experiencing psychotic disorders and complex mental health needs. The role incorporates comprehensive assessment, care coordination, therapeutic intervention, and collaborative work with multidisciplinary partners, carers, and community services. The RN2 provides advanced nursing practice under minimal supervision and contributes to clinical leadership within the team. About the role: This role reports operationally to the Team Manager, and Clinically to the Nurse Consultant and Nursing Director. Duties for the role will include: Developing and maintaining collaborative working relationships with all services within Mental Health and SALHN that supports the provision of an integrated service to consumers, their families and carers Working in conjunction with government and non-government agencies to effectively coordinate service provision Undertaking a clinical case load and providing individual care coordination to consumers with complex and specialised needs utilising discipline-specific expertise Required to, within pre-determined guidelines, and in a multi-disciplinary primary health care setting, assess clients, select, and implement different therapeutic interventions, and/or support programs and evaluate progress About you: Must be highly motivated and have the ability to engage with consumers who have high service needs Ability to undertake comprehensive assessment skills including, consumer needs analysis, assessment skills in mental state and risk assessment Ability to work collaboratively and cohesively with consumers, carers, other agencies, community services and the multi-disciplinary team Experience in the collaborative development, implementation and monitoring of planned interventions for consumers with complex mental health needs Please see the attached Role Description for additional information.
