About the role
Assist NDIS participants in coordinating and implementing support services based on their NDIS plan, using a person-centred approach. Build strong relationships with participants, their families, NDIS coordinators, planners, and service providers to ensure effective support and service delivery.
The successful candidate will have previous NDIS Support Coordination experience to support an existing caseload including complex clients. This role is permanent part time (22.8 hours/3 days per week with scope to increase) role which can be flexibly based in Hobart or surrounds with hybrid work from home arrangements.
Key responsibilities
Strengthen and enhance our client's ability to coordinate supports and participate in the communityRegularly monitor our client's NDIS plan to ensure services are being delivered within their allocated budgetEnsure mainstream services meet their service obligations to clients (i.e. housing, education, justice, health)Provide the client and the NDIA with reports on outcomes and success indicators, within the agreed reporting frequencySkills & experience
Experience in a support coordination roleDemonstrated experience in working with people with a disability and mental health issuesExcellent verbal communication skills and a demonstrated ability to engage with clients and their families to support them as participants of the NDISStrong written and computer skills for the creation of plans, programs reports, correspondence and other documentsEstablished time management, problem solving and conflict management skillsAbility to think creatively and use initiativeUnrestricted driver's licence and willingness to travel as requiredSuccessful candidates will be required to clear probity checks including National Criminal History Record Check, NDIS Workers Screening Check and Working with Vulnerable People Check.
