Navigate Support Coordination is currently seeking experienced and motivated Support Coordinator, Psychosocial Recovery Coach and Specialised Support Coordintor to join our Disability Services team. This role assists NDIS participants to understand their plans, exercise choice and control over their preferred service providers. You will work creatively and resourcefully with participants towards optimal utilisation of their NDIS plans, to maximise opportunities for participants to best achieve their goals. This position is integral in strengthening the participants abilities to self-direct their own supports and actively participate in the community.
What do we need from you?
- Relevant qualification, including but not limited to Cert III, IV or Diploma in Disability.
- National Disability Insurance Scheme Worker Screening Clearance in accordance with the National Disability Insurance Scheme (Practice Standards—Worker Screening) Rules 2018.
- A strong understanding of how and where to access services within the disability industry.
- Experience working a caseload including facilitating personal planning processes.
- Ability to build positive working relationships.
- Experience working within the NDIS.
- Experience writing NDIS required forms
- Excellent time management and problem-solving skills.
- Valid WA driver's license and access to a reliable, insured motor vehicle.
- Provide evidence of COVID-19 Vaccination as required under the WA Government Public Health Order effective 1 December 2021 (Public Health Act 2016 (WA)).
- Technological proficiency and experience with Careview would be an advantage.
