This role involves delivering clinical services, health education, and support to clients in their homes or within the community, promoting health and independence.
Key Responsibilities:

+ Provide comprehensive nursing care, including assessments, treatments, and medication management.
+ Support patients with chronic conditions, post-operative care, and palliative needs.
+ Collaborate with multidisciplinary teams to develop and implement individualised care plans.
+ Promote health and well-being through education and preventative care initiatives.
+ Maintain accurate documentation and adhere to compliance and professional standards.
+ Build strong relationships with clients, families, and community organisations.
Requirements:

+ Current registration with [Insert relevant nursing body, e.g., AHPRA.
+ Bachelor's degree in Nursing or equivalent qualification.
+ Previous experience in community or primary healthcare (preferred).
+ Excellent communication, organisational, and interpersonal skills.
+ Valid driver's license and access to a reliable vehicle.
