About the Role:
An exciting opportunity exists for a passionate and person-centred Grade 3 Occupational Therapist to join the Western Health at Home teams working within the Community Based Transitional Care Program in a permanent full-time position.
The successful applicant will work in partnership with a strongly committed team to and implement high quality and efficient care to patients in the home environment.
Western Health's Community-Based Transitional Care Program is a dynamic initiative supporting older individuals as they recover from hospital stays.
This short-term, goal-driven service helps patients maintain their independence and remain living at home through tailored low-intensity therapy, practical support, and case management—all delivered in the comfort of their homes.
The program plays a vital role in enhancing patient outcomes and ensuring smooth transitions across the healthcare system through supporting care in the home.
About You:
Advanced clinical skills and minimum 5 years' experience in an Occupational Therapy clinical practice role, along with current registration with AHPRA
Experience/qualification in aged care and/or community-based occupational therapy
Ability to independently develop and deliver comprehensive evidence-informed Occupational Therapy service at an individual and team level across diverse client groups
Demonstrated strengths in positively leading others and a solutions-focused mindset
Experience initiating and managing quality improvement and/or research aligned with Western Health's strategic plan
Exceptional prioritisation, time management and problem-solving skills
Strong interpersonal, written, and electronic communication abilities
Commitment to ongoing professional development and lifelong learning
A current Victorian driver's licence and willingness to undergo a police check
