* $45 - $48/hr + Super + Salary Packaging
About the Role
We are seeking a Registered Nurse who is passionate about quality care delivery and education surrounding chronic disease management.
The role of the Chronic Disease Nurse is to work with clients that have been identified as having a chronic disease; including working closely with AWAHS GPs, Practice Nurses and Aboriginal Health Practitioners around the preparation of chronic disease management plans and health checks, involvement in disease specific clinics, referrals to allied health services, referrals to health programs, and regular review of recall register .

To be considered for this position, you will need:
* Current APHRA registration as a Registered Nurse (Division 1)
* Experience working in chronic disease management, developing and managing chronic disease care plans and running effective chronic disease clinics
* Understanding of the health issues impacting Aboriginal and Torres Strait Islander communities
* Confidence using Microsoft Office and electronic client management systems
* The ability to work autonomously and take initiative, while contributing to a collaborative, multidisciplinary team
* A current Working With Children Check and Driver's Licence
* A willingness to obtain a satisfactory Police Check
