The Adult Community Health Service includes Chronic Conditions Management, Community Rehabilitation, Post-acute, and Persistent Pain Management service. The multidisciplinary team works closely with acute services and Primary Care including General Practice and many other non-Government services to enable clients to better self-manage their health. Oral Health, Sexual Health, Home Support Programs, ACAT and Child, Youth and Family Services are co-located onsite. The Cairns North Community Health facility is an innovatively designed facility with multiple services for the Cairns community on the one site enabling accessibility and collaboration.

About your role:
* Demonstrates evidence of increasing autonomous practice and greater levels of professional judgement, influencing practice as a multi-disciplinary team member.
* Promotes cooperation, teamwork and understanding in undertaking generalist and/or specialist nursing/midwifery practices for effective health outcomes.
* Practice collaboratively and provide clinical leadership within the interdisciplinary health care team to promote the safety, security and personal integrity of individuals and groups.
* Provide clinical leadership for professional and clinical practice, education and research in conjunction with the NG7 and Team Leader.
