  • Employment Type: Ongoing, Part-Time (0.6 FTE)
  • Salary: Occupational therapist (Grade 2 VF6 - VF9) + Super + Salary Packaging

About the role

An exciting opportunity exists for a passionate and patient‐centred Grade 2 Occupational Therapist to join the Western Health at Home team within the GEM at Home service on an ongoing, part‐time basis (0.6 FTE). The successful applicant will work collaboratively within a highly committed interdisciplinary team to deliver high‐quality, efficient, and innovative care to patients in their home environment, while contributing to ongoing service development and improvement.

Key Responsibilities

  • Provide Advanced Occupational Therapy Assessment and Intervention
    • Conduct comprehensive occupational therapy assessments and deliver evidence‐based interventions for predominantly older adults receiving care in home‐based settings, particularly within the GEM at Home program.
  • Promote Best Practice in Aged Care Management
    • Apply advanced clinical expertise to support high‐quality, patient‐centred care and ensure assessment and treatment approaches align with current best practice standards for geriatric patients.
  • Support Clinical Leadership and Staff Development
    • Provide professional guidance, mentoring, and support to occupational therapy colleagues and multidisciplinary team members, contributing to workforce capability and continuous improvement.
  • Coordinate Care Within Multidisciplinary Teams
    • Collaborate with patients, carers, and healthcare professionals to develop, implement, and review care plans that maximise independence, safety, and functional outcomes in the home environment.
  • Deliver Services Across Multiple Community Programs
    • Flexibly provide occupational therapy services across Western Health at Home programs as required, including GEM at Home, Community Based Transition Care Program (CB TCP), Rapid Allied Health (RAH), and Specialised Rehabilitation Program (SRP), ensuring continuity of care and efficient service delivery.

Qualifications and Experience

  • Occupational Therapy Qualifications and Registration
    • Minimum 2 years of clinical Occupational Therapy experience.
    • Current AHPRA registration.
    • Eligibility for membership with the relevant professional association.
  • Aged Care and Community‐Based Experience
    • Demonstrated experience and/or additional qualifications in aged care and/or community‐based Occupational Therapy.
    • Ability to support older adults in home‐based settings.
  • Advanced Clinical Assessment and Intervention Skills
    • Ability to independently conduct comprehensive assessments and deliver interventions across a range of diagnostic groups.
    • Strong clinical reasoning and evidence‐based practice skills.
  • Leadership and Quality Improvement Capability
    • Experience managing portfolios, leading initiatives, and contributing to quality improvement activities.
    • Ability to support service development and promote best practice.
  • Strong Communication, Organisation, and Autonomous Practice
    • Highly developed interpersonal, written, and electronic communication skills.
    • Strong leadership, problem‐solving, organisational, and time‐management abilities.
    • Capacity to work independently while collaborating effectively within an interdisciplinary team and managing competing priorities.

Additional Essential Requirements

  • Commitment to ongoing professional development.
  • Current Victorian driver's licence.
  • Willingness to undergo a police check.
