About This Role

We're seeking a compassionate and organised Care Partner to deliver exceptional care services for older adults living in the community.

This client-facing role involves coordinating and overseeing Home Care Package (HCP) services, ensuring they meet each client's unique needs, preferences, and goals.

You'll work closely with clients, their families, health professionals, and service providers to ensure care plans are current, risks are managed, and services are delivered with excellence.


Key Responsibilities Include

* Ensure care and services align with client needs, goals, and preferences
* Conduct regular client reviews and home visits to assess care effectiveness
* Identify and manage risks to client safety, maintaining accurate documentation and records
* Respond promptly to incidents and feedback from clients, families, and healthcare professionals
* Coordinate services with internal and external providers to ensure seamless delivery
* Act as a primary point of contact for clients, families, GPs, and allied health professionals
* Organise family conferences and multidisciplinary review meetings to discuss care progress and adjustments
* Maintain open communication with clients about changes to agreements, care plans, and budgets
* Participate in on-call duties every sixth week, with an RDO provided in lieu


About You

You possess excellent communication skills, a genuine commitment to delivering high-quality care, and thrive in collaborative environments. Your attention to detail, organisational skills, and ability to manage competing priorities ensure clients receive the support they need when they need it.


Required Skills and Qualifications

* Case management experience, preferably in aged care or community care
* A solid understanding of Home Care Packages and sector compliance requirements
* Strong time management and organisational abilities
* Excellent communication and relationship-building skills
* Proficient computer skills
* Clinical registration and experience (e.g. EN, RN), although not essential
