For this role, we provide a direct development pathway toward becoming a Support Coordinator (COS). About the Role The Support Liaison serves as a vital bridge between customers and various service providers. Your primary focus is ensuring a participant’s NDIS plan is understood, activated, and utilized efficiently. Working within the Support Coordination team, you will manage a defined caseload of low-to-moderate complexity, focusing on safeguarding continuity of care and enabling progress toward personal goals. Key Responsibilities Plan Implementation: Support participants to understand and implement their NDIS plans to pursue identified goals. Service Connection: Assist participants to connect with appropriate NDIS providers, community, mainstream, and other government services. Continuity of Care: Maintain effective engagement with the participant’s allocated Support Coordinator to ensure seamless service delivery. Quality Assurance: Undertake quality checks of supports to ensure services align with the participant’s plan, goals, and organizational standards. Stakeholder Engagement: Develop and maintain effective working relationships with internal teams (Customer Service, Business Assurance), the NDIA, families, and health professionals. Documentation & Compliance: Maintain accurate, timely, and compliant documentation across all systems in accordance with NDIS Practice Standards. Performance: Meet or exceed Support Liaison targets and ensure customer processes are carried out efficiently within required timeframes. Your Ticket to Play Qualifications: Certificate IV in Disability or related areas. Experience: A minimum of 2 years’ experience in the NDIS. Interpersonal Skills: Well-developed communication skills to support effective engagement with participants and stakeholders. Technical Literacy: Demonstrated proficiency in Microsoft Office and reporting systems. Resilience: Ability to manage competing priorities across a caseload and maintain professional boundaries in distressing situations. Compliance Requirements: Current valid NSW Driver’s License. National Criminal Clearance Check. NDIS Worker Check and Working with Children Check (WWCC). Valid Right to Work in Australia.
