Overview

Primary Purpose Of The Role: Responsible to deliver patient centred care within the haemodialysis setting in accordance with organisational core values / standards of practice and codes and guidelines as set out by APHRA. Provides support to the State Manager / Dialysis Clinic Manager by participating in the clinical, management, education and quality activities of staff and patients within the clinic. This position will act as a senior team member, promoting the values, mission and goals of the clinic and company, constantly working towards their achievement.

Key Responsibilities

1. Utilises advanced nephrology nursing skills in the design, planning, delivery and evaluation of patient care within the haemodialysis clinical setting.
2. Protects the rights of patients by adhering to Privacy Principles and providing an excellent standard of care.
3. Functions as the primary coordinator of care for a group of dialysis patients, collaborating with other health care providers in developing and implementing individualised plans of care.
4. Responsible for own professional development. Participates in professional development activities, attending both internal and external education programs, to ensure maintenance and updating of professional knowledge and skills for clinical competence.
5. Completes clinical and mandatory competencies annually.
6. Provides education and support to the patient and patient's carers, to facilitate the attainment, maintenance and restoration of optimal health and well-being.
7. Provides support and guidance to level 1 registered nurses, enrolled nurses and PSAs working within the facility and delivers care as part of a nursing team.
8. Performs staff education and assessment, acting as a preceptor and role model as required.
9. Provides education, support and guidance to any visitor in the clinic.
10. Ensures/promotes a safe, secure and healthy working environment to staff and a comfortable, safe environment for visitors and patients as detailed in the Workplace Health and Safety policy (FMEAU-28).
11. Understanding of the Quality improvement process and risk management obligations.
12. Participates in clinic management and business development by adopting a portfolio of responsibilities additional to direct patient care, including but not limited to:
13. quality improvement co-ordination
14. infection control promotion / monitoring
15. occupational health and safety promotion / monitoring
16. Co-ordination / delivery of staff education and professional development programs.
17. Clinical research projects.
18. Actively participates in the quality improvement process by reporting all incidents, hazards and opportunities for improvement.
19. Participates in the review of organisational policies and procedures as required.
20. Depo sites in a higher-level position as appropriate and when requested.
21. Liaises with nephrologists and other members of the allied health team.
22. Maintains accurate patient information in medical records, including Therapy Data Management System (TDMS) and abides by confidentiality and Privacy Policy.

Competencies (attitude, skills, typical qualifications & experience)

23. Essential:
24. Registered as a general nurse with AHPRA (Australian Health Practitioner Regulation Agency)
25. Current practising certificate.
26. Minimum three years current relevant nursing experience
27. Holds / working towards an approved nephrology nursing qualification
28. Ability to continuously reflect on nursing practice and apply learning as appropriate.
29. Demonstrated organisational, decision-making and problem-solving skills, combined with the ability to be self-directed.
30. Computer literacy
31. Demonstrated commitment to quality improvement initiatives.
32. Demonstrated advanced clinical competence and expertise in nephrology nursing
33. Demonstrated ability to effectively perform staff and patient education
34. Effective communication and interpersonal skills including the ability to operate within a multidisciplinary team
35. Self-motivated with an ability to work independently

Position requirements

37. Requires rostered hours of work of minimum 60 hours per fortnight.
