The Transitional Care Professional role is a temporary 3-year position designed to develop high-level clinical expertise and provide support to healthcare services while working towards an endorsed Nurse Practitioner qualification.

Key Responsibilities:
• Conduct thorough assessments and manage clients using advanced knowledge and skills
• Order diagnostic investigations, prescribe medications, and refer clients to other healthcare professionals as needed
• Lead the development of clinical practice by utilizing research, providing education programs, and planning clinical services
• Provide high-quality care for clients, adhering to established guidelines and standards
Requirements
Compliance with applicable vaccination requirements for Category A roles
Working With Children Check (WWCC)
Current AHPRA Registration
Master's degree in nursing or equivalent qualifications
Postgraduate qualifications in drug and alcohol nursing and/or extensive experience in drug and alcohol services
Valid NSW Driver's Licence
