We are seeking a highly skilled Social Worker to join our team. As a key member of our multidisciplinary team, you will provide high-quality social work services to clients and their families, assisting them to cope with the effects of illness, grief, loss, trauma, and difficult life events.

Your Key Responsibilities:
">
* Assess clients' needs and develop targeted interventions to support their well-being
* Provide ongoing clinical assessments and targeted interventions, including risk assessments, grief and loss counselling, adjustment to illness and coping skills, referrals and collaboration with internal and external services
* Work across acute and subacute wards at Warrnambool Base Hospital with the opportunity to have a rotation into community rehabilitation programs as required
* Develop and maintain relationships with internal and external stakeholders to ensure seamless service delivery
* Maintain accurate records and contribute to quality improvement initiatives

Requirements:
">
* Australian Association of Social Workers (AASW) accredited Bachelor Degree or entry level Master's Degree of Social Work that provides eligibility for membership of the AASW
* Excellent communication and interpersonal skills, with the ability to work effectively in a multidisciplinary team
* High-level clinical assessment and intervention skills, preferably within the public health sector
* Skills in psycho-social assessments, problem solving, communication, and counselling interventions
* Demonstrated commitment to professional development, with a focus on evidence-based practice and continuous learning
