Job Description:
As a Case Manager, you will support children and young people to achieve their case plan goals, including supporting them to be restored to their family, live independently, move to a permanent home or step down to less intensive placement types.
This role involves working with Children & Young People in out of home care and understanding trauma and its effects on children and young people. Regular travel across Grafton, Lismore, Tweed Heads and Coffs Harbour is a requirement of this position.
Case Managers are responsible for developing and implementing high-quality therapeutic case plans for Children and Young People in our care. They work closely with the team to ensure compliance with all internal policy and procedure, statutory and contractual requirements in relation to management of service delivery.

Required Skills and Qualifications:
* A relevant bachelor's degree, or a relevant diploma and be working towards a degree.
* Previous case management experience within an Intensive Therapeutic Care or Out Of Home Care (OOHC) setting.
* Experience supporting Children & Young People with disability, who have experienced backgrounds of trauma, abuse and/or neglect.
Suitable candidates will also have an understanding of current casework practices for children and young people.
