Key Responsibilities:
* Support participants in achieving their goals, particularly in alignment with their NDIS plan, emphasizing safety, dignity, and client objectives.
* Provide in-home support with health requirements, personal care, and daily living tasks tailored to individual needs and Care Team requirements.
* Assist in delivering activities to enhance capacity and skills across physical, social, emotional, and intellectual domains.
* Collaborate with a team of health professionals, family, friends, and carers to implement a program of support, including Behaviour Support Plans and Allied Health Plans.
* Respond to crisis situations, including behaviours of concern as outlined in the Behaviour Support Plan.
