We seek a dedicated and compassionate Support Coordinator/Psychosocial Recovery Coach offering Support Coordination and Psychosocial Recovery Coaching for those in our community who require assistance navigating services and supports approved by their NDIS plan.
We’re offering a permanent part-time position for COS (24 hours per week) on Mondays, Wednesdays and Fridays. Leaving Tuesday & Thursday for Psychosocial Support potentially giving you up to an extra 14 hours per week. You’ll start off in our Nerang office, and come April we’ll be packing up and moving into our brand-new office in Buccan, only 8 minutes inland from Beenleigh. New space, same great team!
Key Responsibilities
* Support Coordination
* Working with participants to manage and adapt to the complex challenges of mental health. You will have small caseload plus intake of new clients
* Using a personal recovery approach embedded within a choice and control framework to engage with participants about their psychosocial support services.
* Assist clients in developing coping strategies and skills to manage daily challenges.
* Provide one-on-one coaching and support to individuals in their psychosocial recovery journey.
* Collaborate with clients to identify their strengths, goals, and aspirations.
* Progress reporting and case notes experience
Qualifications and experience:
* Minimum Certificate IV in Mental Health or Diploma in Mental Health or Counselling or Community Services along with experience working with complex Mental Health needs is essential.
* Demonstrate at least 12 months of experience in either Recovery Coaching, Peer Support and Support Coordination
* Experience coordinating a broad range of community service programs.
* Proven experience in supporting individuals with mental health challenges.
* Strong understanding of psychosocial recovery principles and approaches.
* Excellent interpersonal and communication skills.
* Drivers Licence – C class, as well as your own registered vehicle
* Current National Police Clearance or willingness to obtain
* NDIS Worker Screening or willingness to obtain
* Working with Children’s Check or willingness to obtain
