We have an exciting opportunity for an experienced Primary Care Liaison Coordinator to join our future focused and rewarding organisation.
Darling Downs and West Moreton PHN is a not for profit organisation that works at a regional level to achieve improved whole of system health care.
Our key objectives are to:
* increase the efficiency and effectiveness of health services for patients—particularly those at risk of poor health outcomes, and
* improve coordination of care to ensure patients receive the right care, in the right place, at the right time.
Our PHN achieves this by working with local health care stakeholders and community to:
* fund programs and services based on local health needs;
* support primary health care professionals, like GPs, general practice staff and allied health professionals, to improve the care they provide to their patients; and
* support the development of a local sustainable health care workforce.
About the role
Your purpose is to contribute to the primary care activities across the Darling Downs and West Moreton Primary Health Network (PHN) region to coordinate specific programs/projects approved in our Activity Work Plans.
Key purpose of the role
Within a place-based approach you will be required to:
* Establish strong and effective partnerships across general practices, allied health and pharmacy, service providers and other key stakeholders.
* Provide consultancy, advice and strategies to general practitioners, allied health practitioners and other primary health care providers in evidence-based quality improvement approaches, business sustainability, clinical capacity and integrated team-based care.
* Project manage and lead the implementation and ongoing monitoring and evaluation of various primary health care projects.
* Remain up to date with current trends and developments in primary health care and identify, develop and disseminate appropriate resources including data and digital tools and health platforms for Medicare reform, quality improvement and team-based care.
* Provide coaching and support for General Practice and Allied Health on business and quality improvement activities including accreditation, MBS Billing, Primary Sense and data analytics to support business functions and patient care.
* Facilitate the adoption, rollout and implementation of various data audit and extraction tools, and digital health strategies and platforms in the General Practice and Allied Health environment.
* Contribute to PHN workforce development activities.
* Attendance at and assistance in facilitation of education and networking events including chapter meetings and feedback forums.
* Identify areas of workforce shortage and provide support for attraction, retention and upskilling of local workforce to meet the needs of the community.
* Create opportunities for collaboration for community and allied health professionals with General Practitioners, HHS's, Hospitals and other service providers.
* Monitor and report on engagement activities and outcomes, highlighting identified areas for improvement, emerging issues and implement evaluation plans as directed.
* Understand the local health needs surrounding your allocated priority area and be aware of and promote all current and future projects relevant to this priority.
Selection Criteria
* Exceptional communication skills and the ability to effectively present information to Health Professionals and other key stakeholders.
* Demonstrated knowledge and experience of working within an agile environment, including flexibility and ability to prioritise according to the environment.
* Experience in program development and project management to affect systems change including a good understanding of digital and data analysis, project evaluation and reporting in Primary health care.
* Ability to work collaboratively and influentially with a range of stakeholders.
* Highly developed interpersonal and communication skills, including influencing, negotiation and consultation skills, with the ability to build both internal and external relationships. Demonstrated ability to actively participate in a working environment that demonstrates the values of quality, professionalism, teamwork and performance.
