About Us
Disability Plan Services is committed to making the NDIS easier, more empowering and more personal for the participants we support. We are seeking experienced Support Coordinators (three roles) to continue delivering high quality support in the Toowoomba region.
About the Role
You will support participants to understand and implement their NDIS plans, coordinate services, build capacity, resolve barriers and maintain accurate documentation. These roles require strong communication skills, sound judgement and the ability to work independently while managing a varied caseload.
What We're Looking For
Minimum 12 months experience in Support Coordination, Specialist Coordination
Strong understanding of the NDIS, plan implementation and compliance requirements
Ability to manage competing priorities and work autonomously
Relevant qualifications in Disability, Mental Health, Community Services, Social Work or Allied Health
Driver's licence and reliable vehicle
NDIS Worker Screening Check and Blue Card
NDIS Orientation Module, NDIS Effective Communication Module
