About The Role
This role is for HomeMade, a related entity of Mable. HomeMade operates as a completely separate business, but shares a vision for self-management. We are a health-tech start-up (established in 2020) transforming home care in Australia through a creative approach designed to support people in achieving their goals while living at home. Leveraging our technology platform, we are an approved provider that specialises in self-management. We partner with customers to create unique support plans that meet their needs and optimise their funds, making the most of their Home Care Package (HCP). The role involves working collaboratively and proactively through telehealth to help customers stay safe and independent at home, triaging clinical concerns with care, confidence, and clarity, and providing expert clinical input to guide decisions.

Role Responsibilities
- Deliver on all requirements of the Clinical Roster including monitoring and triaging the Clinical Inbox and wellbeing notes
- Complete clinical assessments and reviews (e.g., VATs, CNAs)
- Support customers and families with informed decision-making
- Guide palliative care and advance care planning
- Respond to incidents and clinical escalations
- Contribute to continuous improvement and clinical quality initiatives
- Provide clinical input across the broader team and provider network
- Align practice with the Aged Care Act, Quality Standards, and the Clinical Governance Framework
Your Skills And Experience
- AHPRA Registration as a Registered Nurse
- First Aid certification
- Experience in Home and Community Care, ideally with aged care clients
- Strong clinical decision-making and case management skills
- A warm, human approach and genuine passion for empowering others
- Excellent communication and customer service skills
- Bonus points if you know your way around self-managed or consumer-directed care
