We are seeking a skilled and compassionate professional to join our multidisciplinary team in providing services to individuals of all ages, working collaboratively with families and stakeholders to achieve positive outcomes.

You will be responsible for building strong relationships with participants, families, and stakeholders, ensuring compliance with relevant guidelines, and conducting comprehensive assessments to identify underlying needs.
You will also design and implement person-centred support plans that promote positive change, provide education and training to carers and support teams, and collect and analyse data regularly to monitor progress and inform ongoing support strategies.
The ideal candidate will have:
* An endorsement against the PBS Capability Framework
* A relevant bachelor's degree (e.g., Psychology, Social Work, Occupational Therapy)
* Full Australian working rights
* Prior experience in community or private settings
* A current driver's licence
* A NDIS Workers Check and Working with children check
