Our Juniper Home and Community Care team provides care in the Community setting and the successful candidate will visit customers in their own homes across the in the Metro and Wheatbelt region.As a Registered Nurse with Juniper Community you will partner with customers to assess their needs, implement strength-based strategies and evaluate effectiveness.
You will conduct clinical nursing assessments, which will include cognitive assessments and care planning.
You will work within the broader Home and Community team including allied health, case managers and community support workers.
Renowned for our commitment to the delivery of person-centred care and best practice in relation to the provision of quality care, you will be working in a supportive environment that encourages initiative.This is a casual position that requires flexibility with shifts scheduled between 8:00 AM and 4:00 PM, Monday through Friday, based on business needs.
