Position: Support at Home Care Partner
Tenure: Permanent Part-Time and full time available
Award: Health and Allied Services Managers Administrative Workers Agreement
About the Role
We are looking for a dedicated Support at Home Care Partner to provide exceptional care and services that enable older individuals to remain safely at home. You will play a vital role in developing, implementing, and monitoring personalized support plans. This role is about one hour from Warrnambool.
Key Responsibilities
* Work within a legislative framework to ensure compliance with the Support at Home Program.
* Guide Health Care Workers to deliver safe and timely care services.
* Actively identify and manage safety risks for participants.
* Collaborate effectively within a team and ensure high-quality service delivery.
* Communicate openly and participate in multi-disciplinary team meetings.
Minimum Qualifications
* Care Management Qualification (or currently working towards)
* Cert 4 Individual Support- with relevent expereince
* Current registration with the Nursing and Midwifery Board of Australia or similar
* Valid Victorian Driver's License
* Current Police Check
* Current AHPRA registration (if required)
* Influenza Vaccine (Current Year)
Key Selection Criteria
Essential:
* Strong time management and prioritization skills.
* Experience in individualized support within a person-centered framework.
* Ability to identify and manage participant safety risks effectively.
Desirable:
* Familiarity with software such as e-Tools, TRAKCare, RiskMan, and Microsoft Excel.
