This is a Recruitment Pool offering Fixed-Term and Permanent positions working 76 hours per fortnight. For Fixed-Term positions, there is the possibility of extensions and/or permanency.
These positions are based at the Midland Community Mental Health Service which forms part of the Bentley Health Service.

Suitable applicants will be placed in a pool from which appointments may be made when similar vacancies occur. Appointments may be made from this pool until 31st May 2026.
Responsibilities
* Provides Care Coordination to Adult Mental Health consumers and their carers/families inclusive of mental state assessments, risk assessments, care planning, counselling, psychoeducation, discharge planning and referral and brokering of other services and Multidisciplinary team members;
* Actively prioritises and coordinates own caseload including participating in clinical review meetings and case conferences as appropriate;
* Educates consumers/carers in post discharge management and organises discharge summaries/referrals to other services, as appropriate;
* Participates in departmental and other meetings as required to meet organisational and service objectives in addition to participating in ongoing evaluation of clinical practice.
Position Profile & Area
Midland Community Mental Health is seeking suitably skilled and experienced Social Workers for upcoming appointments. In this role you will provide Care Coordination and Social Work services within a multidisciplinary team. Social Workers will facilitate and promote patient safety and quality of care. Practices as a Social Worker as per the Australian Association of Social Workers Practice Standards 2013 and Code of Ethics 2010 and/or the Society of Professional Social Workers Code of Ethics and EMHS policies and guidelines.
