Key Responsibilities
The successful candidate will be responsible for:
* Providing health promotion and education to clients and carers
* Monitoring and promoting health outcomes via Telehealth
* Delivering direct care services across various funding programs, including wound care, diabetes management, medication management, stoma care, and catheter care
* Assessing, developing, and evaluating nursing care through a collaborative approach
* Working with clients to develop and achieve client-centric care plans
* Assisting clients to become independent in managing their health needs through a Wellness and Reablement approach
Requirements and Qualifications
To be successful in this role, you will need:
* Understanding of community nursing and primary health care principles, including consumer-directed care frameworks
* 1 or more years of experience in community, acute, or aged care settings
* Experience working as an independent practitioner or in a team environment
* Strong organisational and time management skills
* Proficiency in using electronic technology, including smartphones, tablets, laptops, email, internet, and client data management systems
* A Division One Registered Nurse qualification with the Australian Health Practitioners' Regulation Authority (AHPRA)
* Experience in providing advanced clinical care, including chronic disease management, palliative care, catheter management, stoma care, wound management, and diabetes management
* Holistic assessment skills and practical proficiency
* NDIS Worker Screening Check or willingness to obtain
* A current driver's license and ability to travel for client care
