Fixed Term Part Time – 56 hours per fortnight

Available from 19th January 2026 to 30th June 2026

An exciting and rewarding opportunity is available for a suitably qualified candidate to join the Chronic & Complex Care team at Portland District Health.

In this role you are responsible for delivering safe and effective clinical care that requires a higher level of clinical decision making that is relevant to your speciality area of practice. You support the development of clinical and leadership skills in staff placed under your responsibility and less experienced nurses, whilst managing an environment that is conducive to person centred care which fosters a culture of engagement and partnership. Short term contract is to cover personal leave, there may be an option to extend should circumstances change.


About you:

* Registered Nurse with Australian Health Practitioners Regulation Agency, and holds a current registration with a minimum of three years' experience in relevant clinical area
* Post Graduate clinical qualification(s) in diabetes education and currently credentialed to practice as such
* Membership of an appropriate professional association - desirable
* Extensive clinical knowledge, skills and leadership experience in nursing.
* Demonstrated sound advanced clinical assessment and analysis skills and knowledge, and ability to implement indicated treatments in speciality area of practice'
* Demonstrated excellent communication skills, written and verbal
* Understanding of relevant accreditation and funding systems, and demonstrated ability to manage continuous improvement portfolio

PDH delivers services from a single campus, employs around 470 staff with an FTE of approximately 290, and has an active volunteer workforce. The People and Culture Unit sits within the Corporate Services Directorate, with other Units including Finance, Building Services, Food Services, Environmental Services, Executive Support, Administration and Health Information.
