As an NDIS Support Coordinator in our personal and effective team at Q Support Coordination, you will play a vital role in empowering individuals with disabilities to achieve their goals and live independently. This full-time position is based in Kingaroy or Toowoomba, Queensland, and offers the opportunity to make a meaningful impact within the local community.

What you'll be doing

- Collaborate with NDIS participants to develop and implement personalised support plans that address their unique needs and aspirations.
- Coordinate and facilitate access to a range of community services, therapies, and assistive technologies to support NDIS participants.
- Advocate for NDIS participants, ensuring their rights are protected and their needs are met.
- Monitor and review the effectiveness of support plans, making adjustments as necessary.
- Maintain detailed records and documentation to ensure compliance with NDIS guidelines and regulations.
- Liaise with key stakeholders, including service providers, government agencies, and carers/families, to ensure seamless coordination of support.
- Working in a small team independently and collaboratively.
- Using our dedicated support coordination software and Microsoft office tools proficiently to provide excellent customer service with professionalism and care.

What we're looking for

- Qualification in a relevant field, such as disability studies, community services or transferable skills.
- Minimum 2 years of experience working in the disability or community services sector, with a demonstrated understanding of the NDIS.
- Excellent communication and interpersonal skills, with the ability to build trusting relationships with NDIS participants and their families.
- Strong problem-solving and critical thinking skills to navigate complex situations and find creative and fit for purpose solutions.
- Commitment to upholding the rights and dignity of individuals with disabilities.
- Ability to work independently, as well as collaboratively within a team environment.
- Proficiency with Microsoft Office Suite (Word, Excel, Outlook) and experience with client management systems
- Valid NDIS Worker Screening Check
- Current National Police Check
- Current Driver's Licence
