We are looking for a Support Coordination Superstar in the Gold Coast and Logan Areas!
**Supports Coordinator.**
Mpower You is based in Ipswich, Brisbane, Gold Coast and Logan. This position will be predominantly working with participants in the Gold Coast and Logan areas. Our office is based at Booval and you will be expected to attend the office at the initial stages of your employment.
We offer a great work-life balance with hours that are flexible, but you must have good availability for phone contact. Once you are confident in the role you will be able to work from home or from our office, but you will be required to travel to participant meetings and stakeholder meetings when required.

You will be supported by a Supports Coordination Manager and a Supports Coordinator admin assistant.
**To be successful in this role, you will have or be able to**:
- **experience as an NDIS Support Coordinator, **with an exceptional understanding of mental health and other disability groups.
- Know how to identify what supports are needed and where to find the services and programs to meet their needs.
- Have a thorough understanding of strengths-based approach and person-centred care.
- You will be able to think outside the square.
- Have a holistic approach to service delivery.
- Possess a high level of both verbal and written communication skills.
- Experience working with people with a disability and, in particular, people with Mental Health needs, Intellectual Impairment and Autism Spectrum.
- You will be able to work creatively and resourcefully with participants in how they utilise their support budgets to achieve their goals.
- Provide access to mainstream, community, informal and provider options.
- Arrange any assessments required to determine the nature and type of funding required (e.g. assessment to determine the type of complex home modifications required).
- Assist participants to understand and decide the budget for each support type and advise any relevant plan manager of the breakdown of funds.
- Experience in providing links to mainstream or community services (i.e. housing, education, transport, health).
- Develop comprehensive reports for the NDIS.
You will also strengthen and enhance participants capacity to coordinate supports, self-direct, manage support and participate in the community.
**You will also be providing participants with assistance to**:
- resolve problems or issues that arise
- help participants and providers to understand their responsibilities under service agreements
- assisting the participant get ready for their plan review
- assess whether they achieved their goals and got value for money for their plan
- identify solutions to problems experienced in implementing the plan
- consider new goals
- helping participants decide on what actions to take to achieve goals in relation to exploring housing options and life transition planning.
**Essential** **requirements;**
- open driver's license
- reliable vehicle that is comprehensively insured
- current police check
- blue and yellow card/ worker screening (or ability to acquire)
- good computer skills with the ability to learn new software programs
- must be able to work independently and self-direct
- ability to multitask
- must be extremely organised
- must have your own computer for working at home
- relevant qualifications such as Cert4 in Mental health or similar.
- In-depth knowledge of the NDIS and experience implementing NDIS plans
- high level of communication skills.
