Role Overview
We are seeking a driven and team-oriented pharmacist to work in a dynamic hospital setting.

The successful candidate will be responsible for delivering modern, patient-focused pharmacy services to patients.
* Prepare Best Possible Medication History lists for hospital inpatients
* Conduct medication chart reviews and medication reconciliation
* Dispense and supply medication and pharmacy products
* Coordinate patient medication requirements for inpatient stay, day infusions and discharge
* Develop and maintain professional relationships with key stakeholders to ensure excellent customer service to clients and patients
Key Responsibilities:
The successful candidate will be involved in various tasks including:

* Coordinating patient medication requirements for inpatient stay, day infusions and discharge
* Developing and maintaining professional relationships with key stakeholders
Our ideal candidate will possess the following skills and qualifications:
* Full general registration as a pharmacist in Australia or New Zealand
* Previous dispensing/clinical experience in an Australian or New Zealand-based hospital or community pharmacy
* Strong knowledge of the Pharmaceutical Benefits Scheme (PBS)
