Your role at St John of God Murdoch Hospital You will provide leadership and direction for the overall coordination and management of the area of specialty, with the objective of achieving best practice outcomes whilst supporting and promoting the Mission and Values of SJGHM. This will be achieved through visible, effective leadership of the service, the standard of nursing practice and quality of patient centred care, the prudent use of resources and the building of effective relationships particularly with key medical practitioners. The Position Demonstrate both clinical and administrative leadership through effective communication, interpersonal skills, change management ability and the development of a high performance team that exhibits a culture of professional accountability and quality service. Promote and maintain collaborative relationships with accredited Medical Practitioners. Actively participate in all relevant clinical and non-clinical committees within and externally to the hospital. Review clinical practice to ensure effective use of available resources. Maintain and interpret service specific data to monitor and improve service quality, provide strategic direction and identify new opportunities for growth. Provide advice on the use of new products or technology. Report to manager each month or at agreed frequency against the agreed suite of KPIs. Maintain advanced clinical skills and knowledge in diabetes management. Assess and facilitate the coordination of care for patients with complex diabetes management issues. Apply advanced problem solving to diabetes management aspects of patient care and assist nursing caregivers to improve their problem solving and decision-making skills. Promote and facilitate a multidisciplinary approach to the delivery of patient care. Provide clinical leadership and clinical consultancy to the multi-disciplinary team, providing guidance and direction to resolve diabetes management issues. Develop, implement and evaluate relevant clinical standards and policies using an evidence-based approach. Initiate and implement research based nursing practice in areas of clinical specialty. Provide education/advice to patients and significant others. Develop education resources and arrange specialised education as required. In conjunction with LOD develop, implement and evaluate in-service education for caregivers. You will hold well-developed skills in communication including conflict resolution, negotiation and problem solving with demonstrated ability to work collaboratively in a multidisciplinary health care environment. You will have knowledge of relevant legislation and professional standards, including Nurses Act, Code of Conduct, Occupational Safety & Health, Equal Employment Opportunity. You will have knowledge of and commitment to quality improvement with demonstrated skills in clinical teaching. To succeed you will hold registration with the Australian Health Practitioners Regulation Agency as a Registered Nurse with post graduate qualifications in Diabetes Education. Current and ongoing status as a credentialed Diabetes Educator (CDE) will be essential for the role.
