Specialist Clinics provide a gateway to the hospital for patients requiring elective surgery and ongoing chronic disease management. Our major referral base is via General Practitioners from all areas of Victoria. This is a fast paced environment where teamwork is essential.**About the Role**An exiting opportunity is now available for a Case Coordinator, can be a Registered Nurse or Enrolled Nurse to join the team on a Permanent Full Time basis, 40 hours per week.The Case Coordinator under the direction of the Nurse Unit Manager is responsible for coordinating patients across the continuum of care from receipt of referral to discharge from clinic, while ensuring compliance with the St Vincent's Hospital Melbourne (SVHM) Specialist Clinics Access Policy.
