Cohuna District Hospital is a welcoming, rural health service located in northern Victoria, dedicated to delivering high-quality care with compassion and respect. Established as a public hospital in 1952, it has a rich history of serving the Cohuna community and surrounding areas.
The hospital offers a range of acute services including medical, surgical and obstetric care through its 8-bed facility. In addition to inpatient care, Cohuna District Hospital provides vital community and home-based services such as District Nursing, Social Support Groups and Meals on Wheels, in partnership with Northern District Community Health.

Role description
Cohuna District Health (CDH) is seeking compassionate, capable and self‑directed Health Care Workers to join our dedicated Aged Care, Acute Services and community program teams.
In this rewarding and hands-on role, you will provide high‑quality personal care and support to individuals in accordance with their care plan and CDH standards. You will work collaboratively with ENs, RNs, ANUMs, NUMs, allied health, medical staff, regional and community agencies, and members of the public to ensure safe, effective, person‑centred care.
Key Responsibilities
* Provide competent personal care in line with the individual's care plan or task list.
* Work self-directed with minimal supervision while maintaining high clinical standards.
* Apply person-centred care and the Reablement Model to maximise independence and wellbeing.
* Support individuals with activities of daily living, lifestyle needs, grooming and dignity.
* Ensure adequate nutrition and hydration support.
* Assist with continence care as required.
* Accurately document care, observations and assessments within scope of practice.
* Report changes in an individual's condition promptly to appropriate staff.
* Follow instructions from the Manager or delegate in relation to care delivery.
* Participate in ongoing care planning and evaluation.
About you
· Minimum qualification for Grade 2:
Certificate III Individual Support and 12 months full time equivalent experience as a health care worker Grade 1 or Certificate IV Ageing Support and six months full time equivalent experience as an HCW Grade 1.
Essential
· Minimum 12 months experience and qualification needed to be considered for Grade 2.
