This role provides high level rehabilitation and aged Occupational Therapy services within the Transition Care Program.
The Role
An opportunity has become available for an Occupational Therapist - Senior (Transition Care Program) to join Gold Coast Health (GCH) where the focus of our 13 000-strong team is on providing world-class health care to people living in the Gold Coast, and northern New South Wales region of Australia.
* Lead high-level Occupational Therapist service delivery across community and Transition Care Program residential
* Provide expert clinical judgement for complex aged care caseloads
* Drive evidence-based practice, service improvement and quality outcomes
* Oversee workloads, delegation and ethical use of brokered services
* Collaborate and lead within multidisciplinary and statewide Transition Care Program teams
About you
To be successful in this position you will require:
* Tertiary degree in Occupational Therapy
* AHPRA Registration
* Evidence of vaccination, or be able to be vaccinated against and remain vaccinated against - Measles, Mumps, Rubella, Varicella, Pertussis and Hepatitis B
* Tuberculosis screening, to identify if the tuberculosis vaccination may be required
* Driver Licence
* Declaration of serious discipline history must be disclosed
* Aged Care Check
* Criminal History Screening will be required for new employees (GCH will cover cost)

* Permanent part time (38 hrs p.f.)
* Base salary ranging from $64.64 - $69.55 per hour + 12.75 % super

Transition Care provides short-term care that seeks to optimise the functioning and independence of older people after a hospital stay. Transition care is goal-oriented, time-limited and therapy-focused. It provides older people with a package of services that includes case management, low intensity therapy such as physiotherapy and occupational therapy, as well as social work, nursing support or personal care to maintain and improve physical and/or cognitive functioning. The programme seeks to enable older people to return home after a hospital stay rather than prematurely enter residential aged care.
