Position Purpose
The Support Worker provides high-quality, individualised care and assistance to clients in their homes, ensuring safety, comfort, and wellbeing. This role involves delivering personal care, social support, and domestic tasks in accordance with client care plans and Guardian Home Support's policies and procedures.
Key Responsibilities
* Provide personal care including showering, grooming, toileting, dressing, and mobility assistance.
* Support clients with meal preparation, light household cleaning, laundry, and shopping.
* Assist clients to maintain social connections and community participation.
* Observe, record, and report changes in clients' physical or emotional condition to the Care Coordinator.
* Promote dignity, independence, and choice in all client interactions.
* Follow infection control, manual handling, and workplace health and safety procedures.
* Maintain accurate documentation and uphold confidentiality at all times.
* Work collaboratively with clients, families, and multidisciplinary teams.

Qualifications and Requirements
* Certificate III in Individual Support (Aged Care, Disability, or Home and Community Care) or equivalent.
* Current National Police Check and Working with Vulnerable People clearance (or ability to obtain).
* Current First Aid and CPR certification.
* Valid driver's licence and reliable, insured vehicle.
