The Primary Care and Community Health Unit consists of 9 centres that provide a range of services for children, young people and their families, adults, older people and their carers. Our services range from health promotion, prevention and early detection through to assessment, treatment and continuing care. Our staff work in partnership with schools, GPs, hospitals, non-government agencies and other organisations to provide care across our centres, in people's homes, other community venues as well as virtual care (telehealth).
Primary Care and Community Health is seeking an experienced and highly skilled Clinical Nurse Consultant Grade 2 (CNC2) to join our Community Health team working Monday - Friday at our Blaxland Community Health Centre and Lemongrove Campus. This role provides expert clinical leadership and supports the delivery of safe, compassionate, and person centred care within a multidisciplinary community health setting.
Working collaboratively with patients/clients, families, carers, and multidisciplinary teams, the CNC2 will play a key role in shaping clinical practice, supporting nursing staff, and improving health outcomes for the community.
As the Clinical Nurse Consultant you will engage in and support education, quality improvement, and research activities. You will provide clinical supervision and mentoring to nursing staff, act as a professional role model, and maintain regular involvement in direct clinical care to support high quality service delivery.

What You Will Bring To The Role
* Current Authority to Practice as a Registered Nurse with the Australian Health Practitioner Regulation Agency and current NSW Driver's License and willingness to use same as required for the position.
* Minimum five years full time equivalent post registration experience, with at least three years full time equivalent experience in Wound Care/Management and post graduate qualifications in Wound Care/Management or such other qualifications or experience deemed appropriate.
* Evidence of consistent demonstration of behaviours that reinforce the CORE Values of our organisation with all stakeholders including colleagues, direct reports patients and consumers.
* Demonstrated clinical leadership skills and an ability to work collaboratively within a multidisciplinary framework with a demonstrated commitment to excellence in nursing practice.
* Demonstrated effective communication and interpersonal skills across a variety of mediums and an understanding of the practice development framework to clinical practice.
* Demonstrated involvement in the development/change of clinical practice underpinned by evidence-based research.
* Demonstrated ability to contribute to strategic and clinical service planning.
* Ability to apply clinical expertise in patient-centred consultancy with the ability to work effectively both as a member of a multidisciplinary team and independently.
