About The Role
As a Care Coordinator, you will assess eligible, high-risk clients living with complex healthcare needs. The primary aim is to provide generic care coordination in ED and the community to facilitate self-management strategies, improve health outcomes, and prevent avoidable hospital presentations.
As an Occupational Therapist, your responsibilities include providing assessment, intervention, and management to support functional recovery, encourage self-management, and promote empowerment. Key duties include:
* Early identification of ALERT patients in the Emergency Department
* Providing clinical management, discharge planning & community liaison for relevant patients
* Engaging with and providing outreach care coordination services for identified or referred clients, ensuring holistic assessment and collaborative care planning
* Provision of clinical interventions such as functional retraining, equipment prescription, home modifications, falls prevention, pressure care, energy conservation, remedial strategies, and referrals for ongoing follow-up
Classification & Salary
Classifications range from Occupational Therapist Grade 2 VF6-VF9 ($90,236 - $103,724 per annum).
Your Contribution
* Current registration with AHPRA as an Occupational Therapist
* Minimum 2 years' experience in relevant acute/community settings
* Current Victorian Driver's Licence
* Current nationally coordinated criminal history check or willingness to obtain

This position is part of the Healthcare for the Homeless Program (HfH), supporting clients experiencing or at risk of homelessness, focusing on complex needs including substance use, mental health, disability, and family violence. The role is based at St Vincent's Hospital Melbourne within the Emergency Department and includes community support as part of an interdisciplinary team.
