Role Overview
This role involves engaging and coordinating service providers on behalf of participants, as well as NDIA staff and Local Area Coordinators. The successful candidate will engage with participants to develop and refine their individual NDIS goals, explain how to use their NDIS plans effectively to achieve their goals, and coach them in capacity building and increasing their independence.
The role requires effective communication with a participant's formal and informal supports to help facilitate achievement of a participant's goals. The successful candidate will manage resources effectively and monitor the participant's progress achieving their individual goals to get the best outcome from their plan.
Key Responsibilities
* Engage and coordinate service providers on behalf of participants, including NDIA staff and Local Area Coordinators
* Develop and refine individual NDIS goals with participants, and explain how to use their NDIS plans effectively
* Coach participants in capacity building and increasing their independence
* Communicate with a participant's formal and informal supports to facilitate goal achievement
* Manage resources effectively and monitor progress
Required Skills and Qualifications
Essential Criteria
* Minimum two years demonstrated experience in a similar role including Mental Health and Crisis Management
* Relevant tertiary qualification or similar (e.g., Certificate III in Disability, Individual Support or Diploma Community Services)
* Current WWCC, NDIS Worker Screening Check and NSW Driver Licence
* Proficient understanding of the National Disability Standards, NDIS Practice Standards and NDIS operating environment
* Demonstrated time management skills and problem-solving skills
* Effective stakeholder engagement and communication skills
Desirable Criteria
* Degree qualification in Nursing, Community Services, Social Work or Social Science highly regarded
* Mental Health training
* Crisis Management training
