Your Opportunity

We are seeking a culturally responsive trauma-informed and experienced Family Support Practitioner to join our Integrated Family Support Service (IFSS) team in Rosny Park. This is a unique opportunity for a practitioner who thrives in outreach-based trauma-informed environments and is committed to early intervention that prevents escalation into the child protection system.

This isn’t just case management; it’s about meeting families where they are both physically and emotionally. You’ll work autonomously in the community building trust and resilience with families facing complex challenges such as intergenerational trauma, mental health, disability and family violence.

This role is ideal for someone with deep experience working alongside Culturally and Linguistically Diverse (CALD) families and a robust understanding of the Child and Youth Wellbeing Framework.

Your Key Responsibilities

- Engage directly with families to build trust and develop plans that create sustainable positive change.
- Partner with parents and caregivers to identify achievable goals that strengthen family safety, wellbeing and belonging.
- Deliver trauma-responsive strengths-based and person-centred case management for families with children at risk.
- Conduct comprehensive assessments, safety planning and regular reviews to monitor progress and outcomes.
- Develop and maintain individualised case plans aligned with the Child and Youth Wellbeing Framework.
- Provide practical in‑home support, advocacy and referrals to appropriate wrap‑around and specialist services.
- Facilitate case conferences and collaborative meetings to ensure a coordinated approach across service networks.
- Work flexibly in community and home‑based settings to engage families facing barriers to participation.
- Maintain high‑quality, accurate and timely case notes, reports and data in client record management systems.
- Build strong partnerships with community organisations, schools and government agencies to improve outcomes for children and families.

Requirements for Success

- A degree or qualification in Community Services, Human Services, Social Work, Social Sciences, Psychology or another relevant field.
- Demonstrated experience supporting vulnerable families with multiple and complex needs.
- Strong understanding of child development, family dynamics and risk assessment.
- Proven experience in trauma‑informed strengths‑based and early intervention practice.
- Cultural competence and commitment to inclusive practice, particularly with Aboriginal and CALD families.
- Ability to manage a complex caseload while maintaining professional boundaries and confidentiality.
- Exceptional communication, negotiation and relationship‑building skills with children, parents and service providers.
- Sound knowledge of local child safety systems, community supports and multicultural services in Tasmania.
- Proficiency in documentation, case management systems and Microsoft Office applications.
- Current unrestricted driver licence with a willingness to travel for outreach and community engagement.

Before starting work with us you will need to undertake a national police history check, qualifications and referee checks, and have a clearance to work with children/vulnerable people.
