Peel and Rockingham Kwinana (PaRK) Mental Health Service teams are seeking applications from passionate Clinical Nurses

This is a Recruitment Pool for Permanent Full Time, Permanent Part Time, Fixed Term Full Time, Fixed Term Part Time and Casual appointments available across a range of services.

Join our dedicated team as a Mental Health Clinical Nurse!

In this role, you'll be an integral part of a multidisciplinary team, bringing your clinical and professional expertise to deliver comprehensive, evidence-based nursing care to our patients. You'll play a crucial role in promoting patient safety and ensuring the highest quality of care. As a Clinical Nurse, you'll practice within your scope, guided by the Nursing and Midwifery Board’s Nursing Practice Decision Flowchart. Your contributions will be vital in creating a supportive and healing environment for those in need of mental health services.

PaRK is a service with SMSH and provides mental health services to adults and older adults living in the communities of Kwinana, Rockingham, Mandurah, Pinjarra and Waroona. The PaRK mental health service is a vibrant and progressive organisation whose mission is to provide safe and progressive patient care within a positive work environment and culture.

PaRK Mental Health Service is divided into 2 catchments:

- Rockingham and Kwinana
- Peel, Mandurah

Assessment Treatment Team (ATT)

ATT are the first point of access to PaRK Community Mental Health Services and operate in Rockingham. The teams provide assessment, acute response and brief intervention for clients aged 18 to 64 years for a period up to the maximum of 10 weeks. The teams are recovery focused and provide clients with intensive, time limited support. ATT work with the people important to the client and their mental health care such as family and carers. Referrals are reviewed daily in the Intake meeting and allocated a case manager/care coordinator as required.

Assessment Community Team (ACT) – Rockingham

The ACT team provides an evidence-based intensive care management service using a flexible team for clients experiencing severe and enduring mental health illness and those that have had difficulty engaging with mainstream services. Specific eligibility criteria: clients mush be suffering ongoing severe and enduring mental illness, and mush have a history of poor, intermittent or chaotic engagement to services.

Clinical Treatment Team – CTT- Rockingham

The CTT team provides on-going treatment and support for people with enduring mental illness through comprehensive assessment, specialist treatment and intervention, and a cooperative approach to care plans and discharge plans. This includes access to specialised clinical psychology assessment and individual and group evidence based psychological interventions.

Early Episode Psychosis Team (EEP)

The Early Episode Psychosis Team (EEP) covers the Rockingham Peel Group Catchment area with clinicians located in both the Rockingham and Peel Adult Community Mental Health clinics. The EEP Program is an evidence based intensive care coordination service dedicated to early detection and intervention for individuals in the early stages of psychosis. The goal is to provide a service that promotes early recovery for people experiencing psychosis using a biopsychosocial model and to provide support to their family and carers. The team is multidisciplinary and made up of clinical nurses, social workers, clinical psychologists, occupational therapists, psychiatric registrars and a consultant psychiatrist who pride themselves on working collaboratively to enhance client care. Referrals are reviewed on a weekly basis on Friday mornings at the EEP Multidisciplinary team meeting. The clinical responsibility for the client remains with the relevant program/adult team until the referral has been accepted and the client allocated an EEP care coordinator.

Intensive Day Therapy Unit

The Intensive Day Therapy Unit (Boodalang) is a sub-acute service providing non-residential Step-up/Step down services through a day program. The target population for the IDTU is consumers between 16 – 64 years who are significantly disabled by mental illness and/or who require intensive treatment and reside in the PaRK MHS catchment area. The maximum length of treatment within the IDTU will be 12 weeks. All referrals to IDTU for admission must be made through PaRK Mental Health Services.

Kwelena

Kwelena is a sub-acute 10 bed residential service managed by a partnership between PaRK Mental Health Services (IDTU), Access Housing and MIND Australia. It provides short term recovery focused residential service for people who would benefit from 24-hour support to avoid a hospital admission or are leaving acute mental health care. The target population for the Rockingham Kwelena facility is consumers between 16 – 64 years who are significantly disabled by mental illness and/or who require intensive treatment. The maximum length of treatment within the Rockingham Kwelena facility will be 4 weeks.

Community Care Unit (CCU)

CCU is a 20-bed residential service managed by a partnership between PaRK Mental Health Services and Richmond Wellbeing. Accessible to individuals aged between 18 to 65 years who no longer require hospital care, and who would benefit from a supportive transitional care environment to develop their capacity for independent living.

It provides high-level support and rehabilitation in the community within a home-like environment for adults with severe mental health issues. The service is focused on individuals who are likely to require mid to long-term support (i.e. greater than six months and on average 18 months).

It is a transitional service, meaning those within the unit will be supported though psychosocial support, peer support and embedded mental health clinical services to maintain and build sustainable living skills and behaviours. It also promotes the capacity and confidence of individuals to transition from this service into suitable accommodation within the community.
