About The Role
We are seeking a Permanent - Part time Registered Nurse to join our friendly and supportive Home and Community Care team. Our team provides care in the community, visiting customers in their homes across the southern suburbs of Perth.
As a Registered Nurse with Juniper Community, you will partner with customers to assess their needs, implement strength-based strategies, and evaluate effectiveness. Your responsibilities include conducting clinical nursing assessments, including cognitive assessments and care planning. You will collaborate with a broader team including allied health, case managers, and community support workers. We emphasize person-centred care and best practices in quality care delivery, fostering a supportive environment that encourages initiative.
This position is Permanent - Part time, working Tuesdays & Wednesdays (08:00 am - 4:00 pm). Candidates with previous experience in Home and Community Care will be preferred.
Key Responsibilities
Assess and review customer needs in consultation with case managers
Conduct clinical nursing assessments (PAS & KICKA)
Perform cognitive assessments
Develop, implement, and evaluate care plans for chronic and complex needs
Liaise with healthcare professionals to deliver quality care
Participate in multidisciplinary team meetings
Ensure compliance with organisation, professional, and aged care standards
Maintain funding mechanisms and monitor expenditure
Key Requirements
Current registration with AHPRA as a Registered Nurse
Ability to relate to elderly customers with empathy and work effectively with health professionals, service providers, and case managers
Strong clinical assessment skills
Ability to work autonomously with excellent communication, organisational, and time management skills
Enthusiasm for professional growth and development
Desirable: experience with IDC, wound care, and dementia strategies
Cultural awareness and sensitivity
Current Australian driver’s license and reliable vehicle

Attractive hourly remuneration: $44.76 - $56.45, plus 20% casual loading
