Odyssey NSW is a leading provider of Alcohol and Other Drugs treatment and support in NSW and our Community Programs provide a holistic range of strengths based and recovery-oriented supports to those seeking hope and change in their lives, including Aboriginal specific and parenting programs.
Working as part of our broader community team across Greater Sydney and the Southern Highlands, and alongside our Residential, Parent's and Children's, and Medical programs, you'll play a vital role in supporting individuals leaving custody who have been affected by AOD.
Odyssey NSW is looking for an innovative and skilled team member who can provide individual and group treatment, case management, assertive outreach, and wrap-around support to help clients reintegrate successfully into the community.
The Case Manager (Post Custodial Support) works within Odyssey Programs to provide specialist support and case management for individuals transitioning from custody back into the community. The role focuses on care coordination through assertive engagement with Alcohol and Other Drug (AOD) services, health providers, housing supports and social services to ensure clients receive tailored support. By collaborating with service providers and monitoring client progress, the Case Manager helps reduce recidivism, improve health and social outcomes, and support reintegration through holistic, coordinated care.
* Relevant VET qualification.
* Or tertiary qualification in relevant field.
* Experience supporting those with AOD support needs, MH support needs and/or other support needs including those exiting custody or in contact with criminal justice system
* Demonstrable experience providing case management or care coordination in the community.
* Demonstrated experience undertaking brief and comprehensive assessment as well as ongoing assessment of client support needs.
* Experience working within a non-government AOD or MH program.
* Computer literacy including understanding of MS Office Suite and use of Client Management Systems.
* Demonstrable communication skills including report writing, case note writing.
* Knowledge of evidence-based treatment, harm minimisation, care coordination and support strategies.
* Hold a current NSW Driver's License.
* Hold or have the ability to obtain a current Criminal History Check and Working With Children clearance for paid employment.
