Hummingbird House, Queensland's only children's hospice, operates as a medically supported 'home away from home' for children affected by a life limiting condition, and their families. The stand-alone purpose-built facility offers 8 guests in care beds, 5 family accommodation suites and a unique 'end of life' suite.

Underpinned by the notion of creating a 'warm embrace' for children and their families, the multidisciplinary team provides 24/7hr scheduled and emergency specialist care short break stays; pain and symptom management; care at the end of life; and psychosocial support programs for the whole family.

With strong connections locally, nationally and internationally with specialist paediatric palliative care services, Hummingbird House is an integrated community-based service delivering flexible child and family centred care.

Your Role as a Community Development Worker

As a Community Development Worker, you will be responsible for implementing the provision of community development approaches to care at Hummingbird House and link needs of children and families to resources within a community development framework.

You will implement a community development approach to palliative care to identify, develop and more effectively integrate support networks for families receiving care for their child.

Key responsibilities of this position include:

* Leading the development, and delivery of evidence-based community-focused services to enhance the quality of life for children, young people and their families and/or caregivers.
* Supporting the development of stronger and more resilient families and communities using ecological, community connectedness and other evidence-based approaches.
* Working with local communities to enable them to become more welcoming and inclusive for people living with serious illness.
* Establishing and maintaining effective liaison and communication with key community groups and relevant health providers
* Engaging with schools across the region, communities and young people to identify and prioritise needs.
* Supporting the implementation community focused orientation across all areas of practice at Hummingbird House.

This is a part-time opportunity consisting of 30 hours per week.

What We're Looking For

* Genuine ability to listen to stories and narratives of children, families, and communities; identify existing resources in communities and linking these accordingly.
* Proficiency in the development, delivery and evaluation of community-based initiatives aimed at working with children, young people, and families.
* Confidence in communicating personal, principle and community development frameworks of practice.

A Valuable Addition, Yet Not a Must-Have

* A completed tertiary qualification in Community Development or relevant.
* Specialist experience or qualifications in death care, grief and loss, or bereavement.

Essential Requirements

* Proof of COVID-19 vaccination status (or as required by legislation and WMQ policy)
* Relevant probity checks required by legislation and WMQ Limited policy.
* Current Driver's Licence and willingness to travel for work
