Community Health Nurse Opportunity
We are seeking an experienced Community Health Nurse to join our team in the Caboolture/Kilcoy area. This role will provide comprehensive and accurate nursing assessments, case management of clients, and implement planned care.
About the Role
* Provide high-quality nursing care to clients in their homes
* Conduct thorough nursing assessments and develop individualized care plans
* Work collaboratively with multidisciplinary teams to achieve optimal client outcomes
* Manage client records and maintain accurate documentation
Required Skills and Qualifications
* Degree in Nursing or equivalent qualifications and experience
* Current AHPRA registration and evidence of recent practice
* Current CPR Certification
* Current National Police Certificate
* Current Drivers Licence and own reliable vehicle
