About the Role
The Community Mental Health Team provides a comprehensive range of services designed to support recovery and promote sustainable community living for people experiencing mental illness.
The Community Mental Health CORE Team is a multidisciplinary team delivering specialist services to adults living in the community. The CORE Team sits within a broader Community Mental Health Network, which includes a specialist Aboriginal Mental Health Team, bilingual mental health workers, and Peer Support Workers — offering a culturally inclusive and person-centred approach to care. This position is located at Camperdown Community Health Centre, Royal Prince Alfred Hospital.
The Community Mental Health CORE Team Model of Care
The Community Mental Health Team (CORE Team) is the mainstay of the Community Mental Health System. The multidisciplinary team includes Nurses, Occupational Therapists, Social Workers, Psychologists, Psychiatrists and Peer Support Workers. Together, we provide a range of clinical services, including intake, comprehensive assessment, care coordination, consultation, and tailored treatment for priority adult populations with mental illness.
Care coordination is central to this role and model of care. It encompasses short-term interventions, longer-term case coordination, and targeted packages of care based on individual needs.
The CORE Team has 3 key functions:
* Providing for those with time-limited disorders who can benefit from specialist interventions.
* Providing care for individuals with complex and enduring mental health needs.
* Providing consultation regarding the care of people with mental health problems to other health professionals – in particular advice to GPs and a triage function enabling appropriate referral by GPs seeking a specialist opinion.
The CORE Team aims to:
* Deliver care in the least restrictive, most supportive manner possible, while prioritising the safety and dignity of consumers and staff.
* Stabilise and enhance social functioning, protecting community tenure.
* Promote physical health and overall wellbeing
* Partner with GPs to deliver integrated, shared care
* Reduce stigma and empower individuals in their recovery journey

Ideal Candidate
* Recognised tertiary qualifications appropriate to profession. Full registration and eligibility for membership to professional association, as appropriate.
* Demonstrated competencies to provide contemporary mental health practice.
* Understanding of and commitment to recovery oriented services and consumer involvement in service provision.
* Current NSW Drivers Licence (P1 and P2 acceptable).
