* You will be delivering evidence-based therapy, including assessment, intervention, and case management, to support customers in achieving a lifestyle of their choice.
* With a person-centred approach, you will empower customers to take control of their own lives and make informed decisions about their care.
* Using the key worker model, you will collaborate with a multidisciplinary team to provide seamless support to customers.
* You will also conduct regular visits to customers in their homes or in our newly built hub, getting to know them and their needs on a personal level.

Key Responsibilities:

* Use the key worker model to collaborate with the multi-disciplinary therapy services team
* Conduct hub-based and community visits
Essential Skills & Qualifications:
* Tertiary qualification in Speech Pathology
* Membership with Speech Pathology Australia
* Right to work in Australia
* National Police Clearance within 6 months
* Driver's Licence
* NDIS Workers Orientation Module
* NDIS Worker Screening check
* Working with Children Check
* First Aid Certificate
* COVID-19 Vaccination Certificate
