Amplar Health Home Hospital is expanding its Transition Care Service to Walkerville — providing another welcoming location for public hospital patients who no longer require acute care. Here, patients can rest and recover in comfort, while helping free up hospital beds for those who are acutely unwell. This expansion builds on our innovative model of care, enabling patients to move from metropolitan and peri-urban hospital wards into a supportive sub-acute setting designed for recovery and rehabilitation.
We are seeking Clinical Nurses with a specific interest in Education or Management for a full time role at our new location.
Your opportunity
* Coordinate patient care and support smooth transitions across care settings.
* Supporting governance, risk management, and service coordination.
* Contributing to operational processes and system improvement.
* Promoting consistent, safe, and effective clinical practice across programs.
* Participate in quality improvement, service development, and multidisciplinary collaboration.
* Provide clinical leadership and act as a resource for nursing teams.
About you
* Registered Nurse with current unrestricted AHPRA registration.
* Minimum 3 years post registration experience.
* Experience or interest in governance, service coordination, or operational management.
* Advanced clinical decision making and critical analysis skills.
* Strong organisational and leadership capability.
* Ability to manage rosters, budgets and resources.
* Commitment to patient‐centred care and continuous improvement.
* Hold current First Aid and CPRcertificates.
* Hold a South Australian Working with Children Check and National Police Clearance (or are willing and eligible to obtain).
* If successful, willing and able to provide proof of up‐to‐date vaccinations.
