Permanent, full-time | 80 hours per fortnight with a monthly ADO


About the Role

This is a unique opportunity for a dynamic and team-oriented leader to step into the role of Team Leader for the Single Assessment Team within Aged Care Assessment Services Grampians. This position is primarily based in Ballarat, but for the right candidate, there may be flexibility to be placed at one of our West sites, including Horsham or Stawell. In this role, you will demonstrate clinical leadership while driving quality improvement and service development initiatives. You will manage operational priorities, support staff, and oversee a varied caseload, all while working collaboratively with hospitals, aged care facilities, service providers, and key stakeholders. The position offers the chance to make a meaningful impact on aged care outcomes across the region while fostering a supportive, growth-focused team environment.


Key Responsibilities

1. Oversee and coordinate day-to-day team operations
2. Complete complex aged care assessments as required
3. Supervise and mentor assessment staff
4. Facilitate medical assessments and specialist visits
5. Lead quality improvement and strategic initiatives
6. Represent the service at internal and external meetings


Skills & Experience

* Tertiary qualification in Health or Social Science
* Qualified in a Vic West SAS core discipline (e.g., OT, RN, SW)
* Proven team leadership and staff supervision ability
* Strong community engagement and clinical judgement
* Skilled in assessment, planning, and problem solving
