Job Description
WorkWell Coordinator will play a pivotal role in driving the programme forward, ensuring alignment across health, employment, and neighbourhood health agendas.
Key responsibilities include:
* Act as the main point of contact for stakeholders including NHS services, VCSE providers, and community organisations to create seamless integration across the care pathway.
* Motivate individuals and foster strong working relationships with stakeholders.
* Apply a strong understanding of the neighbourhood health agenda and commit to a biopsychosocial model of support.
* Demonstrate enthusiasm, self‑motivation, organisation, strategic leadership, and management skills.
* Lead the local evolution of the programme, positioning it as a national model of integrated health and employment support.
