* 2 x Full-Time, Fixed Term positions until 30th of June 2027

About the GOALS+ Program

The Going Out and Living Successfully (GOALS+) Program supports young people to develop the skills, confidence, and behaviours needed to transition from supported care to independent living.

The program offers case management, coaching, and accommodation for young people aged 16–21 who are leaving out-of-home care, experiencing homelessness, or at risk of homelessness.

Your Opportunity

Provide holistic case management and coaching that empowers young people aged 16–21 to successfully transition from out-of-home care or homelessness to independent living within 12 months. This includes delivering tailored support across key life domains—such as housing, education, employment, health, and wellbeing—while fostering resilience, life skills, and community connections.

The Case Manager works collaboratively with young people, their care teams, and external services using a strength-based approach to develop and implement individual plans that promote stability, independence, and long-term success.

About You

You will have case management experience supporting and engaging young people with challenging behaviours, including those impacted by trauma, homelessness, disability, and protective concerns.

A Cert IV or Diploma qualification in Child, Youth and Family Intervention or relevant qualifications including social work, youth work, alcohol/other drugs and/or substantial experience in working therapeutically with young people is required for this role.

* $15,900.00 pa (pro rata) Salary Packaging + Additional Salary Packaging for meals/accommodation
